Acute Visiting Service *

When patients contact their GP requesting a same day home visit for an acute need, the GP’s can choose to refer on to the acute home visiting service. The aim of the service is to reduce inappropriate hospital admissions and enable patients to be cared for at home when it is clinically safe to do so.

What is the Acute Visiting Service?

The acute visiting service supports the NHS by ensuring that:

  • A reduction in emergency admissions to hospitals.

  • A reduction in the need for GPs to leave their practice to visit people in their homes, increasing patient access to GPs at the practices.

  • Quicker referral to hospital when it is needed.

The service can be requested by patients, usually those that are housebound, by their family, or their GP.

The service is ran by the PCN on behalf of the GPs to make sure that the patient is seen within 24 hours.

A paramedic will come to the patients home, and make sure everything is okay and support the patient with any needs. This is based on referrals to the AVS line.
