Occupational Therapy

Occupational Therapy within Worcester City Primary Care Network (PCN) is a vital service aimed at supporting individuals to regain, maintain, or develop the skills necessary for everyday living and working. Our Occupational Therapists work collaboratively with patients to assess their physical, psychological, and environmental needs, providing tailored interventions that promote independence and improve quality of life. This includes helping people manage long-term conditions, recover from injury or illness, and adapt their environment or activities to suit their abilities. Through personalised support, advice, and therapeutic activities, Occupational Therapy contributes to holistic care, enabling patients to participate fully in their daily routines and community life.

Health Priority Programme
Health Priority Program is for individuals that may require more support, often due to complex health, social, or functional needs. Patients will be identified by the PCN, and eligibility will be confirmed by the GP Practice and then booked into the service by the OT Team.

Frailty Prevention
Pre-frailty is defined as having one or two of the following criteria: Exhaustion, unexplained weight loss, weakness, slowness, and low physical activity, decline in function abilities and ADLs. Practitioners will identify these patients during the practice frailty MDT and are to be booked in directly via GP connect.

Hard to Reach Health Checks
The Reach Out Health Checks initiative is designed to proactively engage patients who have been out of regular contact with primary care services by combining an NHS Health Check with a holistic Occupational Therapy assessment. These appointments will either be practice based or at the patients address. Patients will be identified via the PCN and booked in by the OT Team.
