Worcester City Primary Care Network is the collaborative network of all 10 GP practices in Worcester–and we’re here to support our local care homes in delivering primary care services to your residents.

We’ve designed this pack to allow you to quickly and easily access the information you need to work with us, and to ensure we’re able to provide the best possible support to your centre.

At the bottom of this page you can find all the forms you’ll need to provide us with while working with your residents. We look forward to working with you, and we hope you’ll find our service helpful in providing your residents with the best possible care.

In the event of a death: email completed Record of Death notification form to hwccg.worcestercitycarehomes@nhs.net

In the event of a fall: email Post-Falls Review to hwcch.worcestercitycarehomes@nhs.net

New Residents.

 

What you need to do:

Within 24 hours of a new patient’s arrival at your care home, we’ll need the following information from you:

  • CHT Registration Form, fully completed, with GMS1 or GMS3 form

  • Copy of ReSPECT form

  • MAR Chart

  • LPOA - Health & Welfare if held

  • Copy of discharge summary, if patient is arriving from hospital

  • Medical summary from own GP practice, if patient is temporary resident from out of area

What you can expect from us:

If these forms are completed on time, within the first two weeks of your new resident’s arrival, you can expect the following from us as your provider:

  • New residents work-up by ANP or Paramedic (permanent residents only)

  • Medication Review by Pharmacist

  • Advanced Care Planning by Care Co-Ordinator (if not already in place)

Our Services

 
  • If you require a visit from a member of our team, please email hwccg.worcestercitycarehomes@nhs.net with the following:

    • Completed observation forms

    • Full details of reasons for visit - highlighting key concerns you need our team to address

    • Patient details, including full name, date of birth and NHS number

    • Name of staff member making the request

  • Requests for the following medications can be submitted as needed:

    • Diabetic medication

    • Anticipatory medications

    • Older Adult Mental Health medications (e.g. SSRIs and anti-psychotics)

    • Epilepsy drugs

    • Antibiotics

    • Pain relief

  • You will be given a weekly slot to request all non-urgent medications with our Pharmacist or Pharmacy Technician.

  • In the event of a death, you must email the completed Record of Death notification form on the day of the death to hwccg.worcestercitycarehomes@nhs.net
